The physiotherapy treatment of injured knees

Injuring the knee is one of the most common general and sporting injuries and can give long-term problems with pain, stability and functional activities. Physios begin with the subjective examination, asking about the cause of the injury, the amount of force involved, whether the knee swelled up quickly or the knee was unstable afterwards and the person found it difficult to weight bear.

The amount of pain a patient suffers indicates the severity of the injury involved and the particular location of the pain can point to which anatomical structures have been injured. As the knee will be very difficult to walk on in the presence of a fracture these injuries are rarely missed in diagnosis. During the examination the physiotherapist will test the knee structures to look for the cause of the injury.

Objective Examination of the Knee

The physiotherapist will look at the knee and check for effusion by observation or doing the patellar tap test. The knee can swell greatly and be very tight, needing aspiration by a needle. How well the knee can move when not weight bearing is assessed by the physio. Knee extension is the movement of straightening the knee out and flexion is bending the knee. The knee does have a certain degree of rotation but that is rarely checked in the initial period.

The physiotherapist decides where to start treatment by their findings - how much the patient can easily move the knee, how much pain they have and how the knee behaves on testing. Active movement the patient can do themselves and the physio may gently push the joint further to test weakness or guarding. The physio will test the strength of the quadriceps and hamstrings which straighten and bend the knee respectively, seeing how much the pain or weakness affects the knee function.

Manual testing of the knee ligaments by the physiotherapist tells him or her about these important stability structures. The physio levers the knee inwards and outwards to test the medial ligament and the lateral ligament (the collateral ligaments) and pulls the shin bone back and forward to test the anterior cruciate ligament and the posterior cruciate ligament. Manual palpation around the joint and adjacent structures can help indicate which structure has been injured.

Physiotherapy treatment plan

An acutely painful knee is treated using the PRICE technique, starting with protection of the joint if necessary by using a brace to stabilise it. Crutches or sticks can be used to reduce weight bearing on the knee and allow a good walking pattern. Ice treatment, or cryotherapy, is a first line treatment for an acute knee, reducing pain and the swelling which permits increased movement and progression of treatment. A neoprene knee sleeve may be worn to squeeze the swelling and increase stability.

A reduction in swelling and pain allows the physiotherapist to give exercises to improve the knee’s ranges of movement and strength. The largest and most powerful muscles are the quadriceps and the hamstrings. The quadriceps allows knee power for getting up from sitting, going up and down stairs and walking, keeping the knee stable. After the knee copes with exercise on the plinth the physio will move to exercises in weight-bearing and in more active activities.

Proprioception is the body’s natural joint position sense, with the brain sensing the moment to moment position changes of the knee and co-coordinating the muscle actions needed keep the joint within safe bounds. Physio treatment is balance practice, starting with standing on one leg, then balancing and catching balls on a wobble board and finishing with running, changing direction suddenly, hopping and jumping. Once the swelling has settled and the strength and co-ordination ability restored the knee is ready to rejoin normal and sporting activities.

Hypnosis is simply a state of mind of heightened suggestibility or the willingness to follow instructions. It can also be thought of as a state of focused concentration or trance.

The main idea of hypnosis is to create a suggestible state of mind in the subject. And then implants the desired suggestions. Many times, you do not want someone to realize they are being hypnotized.

When you are hypnotizing someone voluntarily, you can simply ask them to a cooperate. Have them try to become aware of the areas where they are tense and focus on them. Get them to concentrate on relaxing.

If you are trying to hypnotize someone without their knowledge, you can still try and make them relaxed and reduce any tension. Insure that there are no obvious distractions, like loud noises or bright lights.

The real secret to hypnosis is to get the subject to focus inwardly and enter that state of suggestibility or trance. once the subject is in the proper state of suggestibility or trance, you can begin implanting suggestions.

If one had been playing soccer for example, and just received a significant blow to a shin, a few drops of the pure oil rubbed into the area several times over the next 12 hours can make a significant difference in healing time. Creating your own formulas for long-term use is a very easy process. Simply start with your desired concentration — 10% for example. This means one part of Helichrysum essential oil in nine parts of carrier oil. In other words, one milliliter of Helichrysum (27 drops) in 29 milliliters (1 fluid ounce) of the carrier oil of your choice. This can be most easily done by using an empty one ounce bottle with an eye-dropper top. Just add the desired amount of Helichrysum, then fill the bottle the rest of the way up with the carrier oil.

In addition to its pain-relieving action, the pure essential oil includes natural constituents which actually speed wound healing. It is thought that this is a result of the oil quenching free radicals in the injured area, and signaling tissues to regenerate. For these reasons, Helichrysum is also found in many formulas for healing and anti-aging of the skin. Helichrysum is also excellent for burns, and just a little bit of oil on small burns can provide immediate relief. (If using for injuries with broken skin, avoid use if excessive bleeding is an issue, as the oil acts as an anticoagulant. Use only once bleeding has stopped completely, and is not likely to be of concern again in these cases).

The finest of Helichrysum essential oil varieties is considered to be that distilled from plants grown on the island of Corsica, France. The environmental conditions and sub-species genetics result in a unique natural chemical composition of the essential oil with strong anti-inflammate, analgesic and regenerative properties. This oil may be listed as Helicrysum italicum sub-species ’serotinum’. You may find that other varieties work as well for your needs, and can experiment as you see fit.

Helichrysum essential oil is one of nature’s most profound healers. It works for a great many symptoms of pain, safely and effectively. It is simple to use and readily available through internet sources (it is, as of yet, difficult to find in local health food stores, though this may be changing soon). Your Circadian Rhythm -Is It Keeping You Awake?

Your circadian rhythm and it’s level of function is closely connected to insomnia. While it is commonly believed that the circadian rhythm has to do with the time of day, it has a lot to do with the amount of light or darkness. We all have a clock inside that gives us a nudge when it is time to go to sleep for the night and when it is time to get up.

This internal clock within our body controls what is known as the circadian rhythm. These rhythms are associated with the regulation of many hormones that are released within our body throughout the day. For example, our body’s temperature and overall alertness are key factors directly affected by our circadian rhythm.

Since the circadian rhythm influences alertness and our energy level, for those with have a sleep disorder related to their circadian rhythm, their internal clock is “off” and they tend to feel alert when they should be tired or sleepy, and tired when they should have much energy. Does Your Child Have An Itchy Scalp

Do you think your child has head lice? Have you noticed him scratching his head frequently? Or maybe you’ve heard of an outbreak of head louse at his school? You can probably discover if your son or daughter has head louse by doing an easy evaluation at home.

The simplest way to identify head lice is through identifying their eggs in the hair. Lice eggs are called nits. Head lice will lay these eggs on the hair shaft close to the scalp. This is why a head examination requires looking at the scalp. Before the baby lice hatch, nits look tan, brown, or yellow. Once the nit has hatched, the shell will stay attached to the hair shaft. This is the easiest thing to see, as it is whitish, looking somewhat like dandruff. The main difference between head lice eggs and dandruff is dandruff will easily shake off of the hair shaft, while lice eggs are fused to the shaft.

Sometimes you will be able to notice adult head louse on your child’s hair, particularly if your child’s hair is short. These little beasties are small, the same size as a sesame seed from your hamburger bun, and they tend to possess the same coloring as your child’s hair. A lot of parents grasp their little ones are infected the moment they see the adult lice moving through their kid’s hair.

Why am I so Dizzy?

It it facsinating to watch gymnists who gracefully run, soar into the air, flip their bodies around and then land on two feet. While most of us are not trained or equipped to accomplish such feats, many of us perform our own amazing balancing acts daily just by walking on both feet.

Healthy people can do most of that thanks to our amazing balance system.

But unfortunately, the balance system does not always function right and when it stops functioning we experience a feeling called vertigo or dizziness (I’ll talk about it as the same thing here because the cause is the same although these two feelings are experienced in a different way).

We’ve all felt dizziness and had vertigo at some time in our life. Be it from sailing in bad waves, driving on a dusty road, traveling to a high altitude or even having a few too many at the bar (I could tell you some stories).

Fortunately, the feeling doesn’t tend to last long for the majority.

There are people who don’t have the luxury of knowing the feeling is temporary. They will be afflicted with chronic vertigo or dizziness every few months or even every day.

This kind of chronic vertigo/dizziness is caused by problems with the balance system.

Luckily, we can still go about most of our activities even if one of the balance systems is not functioning properly.

However, if two or even three of those balance systems become ineffective, we have a big problem. That is why it is so important to strengthen all three balance systems even if one or two are too weak to completely heal them.

Getting the balance systems strong involves some choreography if you will.

You see, 90% of the time, dizziness and vertigo are caused by tension in the muscles around our balance system - the eyes, the ears or the body line from the top of our skull to the pelvic bone.

Eye muscles suffering from tension will create a somewhat different balance perception than the ear’s balance system is perceiving.

These two balance systems send two different messages to the brain. This causes confusion when your brain tries to process the information. Needless to say, you feel dizzy.

If your reading glasses are not the right strength, you’ve probably experienced this.

A perfectly functioning balance system in the ears and body would help you continue functioning when the eyes are not up to par. However, normally, dizziness still occurs because it just is not likely that both of the other systems will be in perfect balance.

If the muscles tied to each balance system are loose and strong though, you can function properly.

You do not even have to do this perfectly. If you only bring each of these systems to 70% function, that is enough for you to never feel dizziness or vertigo again. That is exactly what my natural vertigo and dizziness program does.
